COACHING CONSENT FORM

Grace Life International (GLI) is a non-profit, non-denominational Christ-centered teaching, training, and discipleship counseling organization. In our coaching, the Holy Spirit is the agent of change - provided the individual cooperates with Him. Our goal is to convey the way of freedom from mental and emotional symptoms, family and marital conflict, law-filled living, spiritual strongholds, and other difficulties, utilizing the Scriptures.

As part of your ADT fee, you are entitled to 5 sessions with your assigned coach.

After the 5 included sessions, Should you choose to continue with your Discipleship counselor, there are two options: Weekly counseling or monthly coaching. Weekly counseling is based on a sliding scale or monthly coaching, is $125.00 per 50-minute
Session.

Since we rely upon the Lord to provide for GLI’s needs, prayerfully trust God as you consider your opportunity/responsibility in this matter. This organization and its operations are funded by the contributions of those being discipled. If you give more than $125.00, that amount will go towards the salary of those counselors who live like missionaries on support.

Since we employ and train our personnel, we do not look at secular standards as necessarily qualifying people for ministry at GLI. Some of our staff have professional, advanced degrees and some do not. I, therefore, understand that the staff of GLI are not professional or licensed counselors, therapists, medical or psychological practitioners. I deem these persons leading these sessions to be “disciplers” in the Christian faith, who are helping me to assume my responsibilities in finding freedom in Christ.

I am also aware that my discipleship counselor/coach may need to intervene if he/she suspects that a child (under age 18) or an elder (over age 65) is currently endangered by abuse or if I am a danger to myself or to others. All sessions will be otherwise confidential. Finally, we train new staff by letting them observe discipleship counseling sessions. Therefore, you may have a staff member and a staff trainee in your sessions.

I understand that I am free to leave at any time and am here voluntarily.

Your appointment is important to us and we ask that you also make it a priority. If you need to reschedule your appointment, please give your coach at least 24 hours advance notice (illness or emergencies are exceptions).
